Help writing apology

First, excuse my bad English. I'm an immigrant, I moved a year ago to live in the UK. I also struggle with comination of mental health disorders: BPD, depression and dicossiation. Today I had my first job induction here, and it was disaster! I was late 20 minutes because it was a long journey by bus, two hours and extra 10 minutes walking. However, I apologised and they let me in.
There was about 10 other candidates in the room, I started filling the form and soon I started to have negative ideas, when the interviewer started face to face interview, I moved towards his colleague table and said:"I'm sorry. I give up the application for the position, I didn't fill so many details. Then the Boss intervened, he asked me "you're not interested any more?" I said I'm interested, "Do you struggle with English? " he asked "Yes " I replied.
But it wasn't the true reason. It was a panic attack. I stopped medication months ago and was depressed over the last few weeks.
I need your help to write an apology letter for the mess I did.
