How can I keep my disability a secret when my project manager asked me to do something I can't?

I am physically disabled, but my coworkers and project manager aren't aware of it. I work remotely and started with my company before any of them, so as far as I know only my project manager's boss and HR are aware.
I like it this way because as far as they know I am a healthy individual, and my work is judged accordingly. I like knowing that my work is adequate without the excuse of a disability. I am also a very private person.
My project manager stated today that for our next sprint meeting, which will be held over a video call (we usually have voice-only calls), we will all need to make note cards so that we can "vote" on upcoming tasks. But I definitely can't make note cards, and I can't hold them up either.
How can I proceed while disclosing my disability to as few people as possible? I considered disabling my webcam drivers and saying my video just isn't working today, but I don't like the duplicity. Worst case scenario would be having to tell everyone during the meeting. Can I just say "I can't do that" and leave it at that?
