I worked as contractor for a staffing firm, how do I put it into the background check form?

Currently I am a contractor for a staffing company. I got a full time job and they want to perform a background check. I am little bit concerned about what should I put down to the form. I am paying by the staffing company, not the client, basically the client has no hiring relation with me. Should I simply put down the staffing company?



Also, in every stage of the interview, I made it clear that I am a contractor, not a full time employee, there shouldn't be confusion in this. I am concerned if they want me to prove this, which I don't know how.
